The information is collected primarily for the purposes of enabling Loop Life to update and maintain a valid marketing and communications database in order to facilitate the transactional processes between Loop Life and the client.  By submitting the information, you hereby confirm the following:

• The information is accurate and complete.
• You have obtained the required consent in order to furnish Loop Life with any personal information of which the designated participant is not the sole beneficial owner.

Loop Life is entitled to process/ and or disclose the personal information if such processing and/ or disclosure is required in order to comply with any applicable law, subpoena, order of the court or legal process served on Loop Life, or to protect and defend its right or property.
